Hypertension

High blood pressure (hypertension) is a risk factor that can increase your chance of developing heart disease, stroke, or other serious conditions. Treatment involves changing various lifestyle risk factors such as diet, exercise, alcohol consumption and smoking, and if necessary, taking medication that can lower your blood pressure.

If you are on hypertension medication, you may be required to attend an annual health review via the Practice’s birthday month medication review system.

A typical annual hypertension review with a member of our Nursing Team will last around 30 minutes.

The review consists of:
• Taking your blood pressure
• Measuring your height/weight
• Taking blood tests where appropriate
• Discuss your overall wellbeing and any symptoms you may have.

If there is a need, we will book you a follow up appointment with your GP or a member of our Nursing Team. This appointment can sometimes be done over the phone, but we may also ask you to return for a second face to face review.
